Michigan HOSA Chapter Memorandum of Understanding (MOU)

As of August 1, 2026, Michigan HOSA requires each chapter to complete and sign the Michigan HOSA MOU in order to participate. Once the completed MOU is uploaded to THIS FORM, chapters will be granted access to the HOSA Conference Management System (CMS). Until then, chapters will not be able to affiliate members for the year.

This Memorandum of Understanding (MOU) establishes the expectations and responsibilities between Michigan HOSA–Future Health Professionals ("Michigan HOSA") and the participating school, its designated HOSA Advisor(s), and school administration for the purpose of establishing and maintaining an active Michigan HOSA chapter.

By signing this agreement, the school acknowledges its commitment to supporting a successful HOSA chapter and ensuring compliance with Michigan HOSA and HOSA–Future Health Professionals policies and procedures.

STEP 1: Download the pdf version of the MOU. Have all applicable staff and administration review.

STEP 2: Open the MOU pdf with Adobe Acrobat. Fill in the school information, initial each line item, and sign. Digital signatures are acceptable.

Print a physical copy of the MOU, fill in the required information, then scan or upload photos of the completed form.

STEP 3: Upload the completed form/file to THIS GOOGLE FORM.

STEP 4: Michigan HOSA staff will review the completed MOU, then grant the advisor access to the HOSA CMS to begin affiliation. Please give MI HOSA staff 1-2 business days to review the MOU and unblock the CMS. An email will be sent when the review and unblocking is complete.
